    Please reschedule your appointment or change it to a telehealth appointment if you are COVID-19 known positive, have a household contact positive, awaiting test results, or known exposure.

    If you have experienced any of the following symptoms in the last 48 hours, please call your provider: fever/chills, cough, shortness of breath or difficulty breathing, fatigue, muscle or body aches, headache, new loss of taste or smell, sore throat, congestion or runny nose, nausea and/or vomiting, or diarrhea.

    Everyone will be screened for COVID-19 symptoms and all persons must wear a mask upon entering the building. Follow social distancing guidelines. Unless you are accompanying a minor or are an essential caregiver, we are not allowing visitors at this time.
